File a Claim

Before you begin, please review the following information to determine which filing option applies:

Individual claim

Use this option if:

• You are filing for yourself as the Claimant
• You are an Attorney filing on behalf of a Claimant or the Claimant’s authorized Agent
• You are an authorized Agent (e.g., family member, etc.,) on behalf of one or more individuals

Under the individual claim filing option, you can submit multiple claims within the same account. For example, you file for yourself and on behalf of others (family members) for whom you are an authorized Agent

Attorney bulk filing

Use this option if:

• You are an Attorney or other authorized employee of a law firm, filing on behalf of more than one client using a cover sheet and provided template
